Your needs will all be discussed and established during the at-home assessment process. For fast-track situations, an initial meeting could also be arranged via a video call, followed by the at-home assessment. At Eaton, our team is trained in advanced care planning to ensure we write a person-centered and holistic care plan for each person we support.

You may not know where to turn if you are a daughter or son in a caring role. Perhaps you are a GP or other health professional.

This service is truly exceptional:

Palliative specialists can not only offer help and guidance to appropriate services, where necessary, they’ll transfer you to another healthcare professional. This may be to someone who can better help you or complete essential referrals to other
services.

Your call puts you in direct contact with one of the region’s Clinical Nurse Specialists, whose role is to make you feel as secure and confident as possible in the care you are receiving.
